Camper Forms

The following forms must be submitted for ALL campers. Campers are not registered until we recieve completed paperwork-

• Registration and health history packet
• Health exam form
• Camper needs assessment

• Parent Handbook- everything you need to know!
• Pick-up authorization form  (must be filled out and returned to office for anyone other than parent or guardian listed on the registration)
